Care-Leaver Link Worker

Isle of Wight Youth Trust is an independent local charity, working in our local community for 40 years, delivering confidential, professional counselling, therapeutic and wellbeing services for children and young people.

If you want to make a positive difference in the lives of children and young people, we want to hear from you. We are looking for a motivated, dynamic support worker, with either lived experience of, or sound understanding of the challenges faced by care-leavers.

Purpose: To coordinate extended support for care-leavers, transitioning to independence. To be the first point of contact for service user/care-leaver referrals, acting as a Trusted Adult, and providing support around employability and independent living, in coordination with the wider wellbeing and therapeutic teams. To carry out needs assessments and refer people to the correct internal service, and to accurately manage incoming data and evidence outcomes.

Experience: You will have experience working in a support worker/trusted adult capacity, collaborating with partnership agencies, and a robust knowledge of the issues faced by those leaving the care system.

Contract type: Fixed Term - 12 months

Hours: part-time -16 hours (2 days)

Salary: £24,570-25779 FTE (pro-rated)
